Mueller communicates: "On a blue plate taken by J. D. Mendenhall and myself on May 13 UT with the 1.2-m Oschin Telescope in the course of the second Palomar Sky Survey, I have discovered a supernova (mag 18.5) located in an anonymous galaxy at R.A. = 16h17m09s.7, Decl. = +17 45'56" (equinox 1950.0). SN 1991ad is 11".4 east and 2" north of the galaxy's center. A CCD frame taken with the Palomar 1.5-m telescope by J. R. Mould on May 17 shows a stellar image of mag 18.5 at this position. N. Reid notes that the object is not visible on a recent Sky Survey plate. A spectrum obtained by W. Baan and J. Salzer with the Hale 5-m telescope (+ double spectrograph) on May 21 shows broad absorption features at 570 and 615 nm, indicating a type-Ia supernova. The redshift of the galaxy is 20 900 km/s."
